change SubsystemHandler/SFTPServerInterface API
change the API of SubsystemHandler to accept a reference to the
ServerInstance object during construction.  this will break all code
that currently creates subsystem handlers (like sftp servers) -- sorry!

lots of little doc fixups (mostly indenting).
